SCOPE OF WORK Experience in the same field of SOW: 5 Years of Experience at least in the SOW (Preferably with evidences). - Valid Driving Licenses & Certificate for Drivers:All Drivers shall hold a valid Driving certificate with the same level for the driven vehicles requirement; - Proof of ownership/leasing for the Vehicles.: - Samples of a valid documents to be provided as proof of Ownership/Leasing for the vehicles to be used during the contract period. - Age:Driver’s age should be in the range of (21 to 65) years old. - Language: Drivers should have at least basic communication skills in English language - HSE: Bidders should pass the minimum requirement of HSE documents as per HSE appendix E. HEALTH-SAFETY-ENVIRONMENTAL (HSE) MINIMUM REQUIREMENTS Vendor shall provide the following: · Preliminary HSE Risk Assessment or Hazard Identification related subject scope · Qualifications and trainings for: o HSE Personnel o Team Leaders and discipline/Trade Supervisor/Coordinator · Vendor’s HSE statistics for each of the last 3 years with details of method of accident and incident reporting: o Number and type of injuries o Number of fatalities o Total worked man-hours o Lost time injury frequency (LTIF) o Total Recordable Injury Rate (TRIR) · Preliminary list of PPE intended to be used with relevant certification standards Administrative Documentation 1. Place of incorporation and details of the entity that will be submitting an offer in case Tender will take place, plus details of any Parent Company and Ultimate Holding Company; 2. Following data from last 3 years financial accounts, even by way of declaration. o Share capital; o Net Assets; o Short-term assets (current assets); o Short-term debts (short-term liabilities); o Turnovers. 3. Number of registered employees up to date. 4. Main declared activity. 5. Name of the Managing Directors and of the person entitled by power of attorney to sign Contracts on behalf of your company. 6. Details of any litigation, in progress or within last five (5)-years, for a prospective claim value in excess of United States Dollars, Five Hundred Thousand (US$ 500,000). 7. Shareholding structure/chain, from the entity that will be submitting an offer (in case of Tender will take place) to the Ultimate Beneficial owners, possibly by providing a chart showing each level of ownership up to the Ultimate Beneficial. 8. “Eni’s Suppliers Code of Conduct” and “Compliance declaration” signed by a duly authorized signatory/representative. In case of a Joint Venture/Consortium, or other type of Grouping, such documents shall be signed by all Parties in order to be legally binding upon each and all of them.
