Evidence of payment of tax (tax clearance certificate) for the last three years. 4. Verifiable evidence of three (3) similar work/activities successfully executed in the past. 5. Recommendation letter from three reputable organizations attesting to satisfactory performance of similar services provided. (This should be consistent with the documents listed in point 4 above). – contact person, email address and phone number should be provided on the recommendation letter. 6. Completed pre-qualification questionnaire. Organizations must send an email to ng-grantsapplication@msh.org requesting the prequalification questionnaire no later than 5:00pm (Nigeria Time) June 05, 2023. Responses to this REOI containing the required documents and pre-qualification questionnaire must be received via this email address ng-grantsapplication@msh.org no later than 5:00pm (Nigeria Time) June 13, 2023.
