Tender Description:

1) The subject of the order is the provision of occupational medicine services for police officers and employees of the Police Headquarters in Nowy Sącz, the Police Headquarters in Tarnów and the Central Police Investigation Bureau, Police Internal Affairs Bureau and the Central Bureau for Combating Cybercrime performing official duties in the area of ​​operation of the above-mentioned Police units, as well as if necessary, police officers and employees of other units Provincial Police Lesser Poland - depending on the selected task. 2) The Ordering Party requires providing services in the city of Nowy Sącz for task no. 1 (KMP in Nowy Sącz), and in the case of task no. 2 (KMP in Tarnów) in the city of Tarnów. 3) Ordering Party divided the subject of the contract into tasks in accordance with the provisions in point 4. 4) Detailed the specification of the subject of the order and the expected quantities of services are included in the annex No. 2 to SWZ - price form and Annex No. 4 - draft contract. 5) Service will be provided provided depending on the needs of the Ordering Party. Expected amounts of medical services given in Annex 2 to the SWZ (price form) are forecast values. These values ​​are indicated only for the purpose of selecting the most advantageous offer by the Ordering Party and do not constitute an obligation of the Ordering Party towards the Contractor. At the same time, the Ordering Party guarantees the performance of the contract at a minimum level. 70% of the contract value specified in §7 section 2. 6) The Contractor guarantees that all tests will be completed within a maximum of 3 business days, taking into account the day the policeman/employee reported to perform the tests. Policeman/employee A person applying for control tests must have them performed together with their issuance decision on the day of reporting - provided that you report by 12:00. 7) In accordance with the provisions of §9 section 8 of the draft contract, the Ordering Party does not require that the tests be carried out within 3 working days counted one by one. The ordering party requires execution all tests did not exceed a maximum period of 3 business days, with possibility of prior registration in person or by telephone. Indication of the date tests in such a case must be included within 14 business days of notification, and the duration of the tests cannot exceed the required 3 business days. 8) After selection the most advantageous offer, and before concluding the contract, the Contractor will present: a) a schedule availability of medical staff and technical potential after selecting the most advantageous offer and before concluding the contract. The schedule must be consistent with the terms of participation in the proceedings; b) unit prices of tests included in test packages (in tables 1-3, referred to in referred to in §3 section 2 of the contract template). 9) The Ordering Party informs that in accordance with Art. 79a of the Act o drivers of vehicles, unit price for conducting a medical examination of drivers professional and drivers of Police emergency vehicles may not exceed PLN 200.00 PLN for each of these tests. In the event that this study is valued at an amount higher than indicated The Ordering Party will treat this action as another error referred to in Art. 223 section 2 point 3 and will improve the unit price of this test.
