At 29°38'40.3"S 30°20'34.2"E Service Providers are required to submit the following documents: - Central Supplier Database Report (CSD) dated within 14 days prior to the close of bid Proposed. - A copy of the company’s Unemployment Insurance Fund (UIF) registration. - Letter of good standing from the office of the Compensation Commissioner and registration certificate as required through the Compensation for Occupational Injuries and Diseases Act (COIDA). - Valid CIDB registration certificate with a CIDB Grading of between 2GB - 4GB. - A valid original B-BBEE Certificate for each participating company or individual professional - Company details (where relevant), mission statement and policies with an indication of the management structure, communication, and supervision. - Proposed model (methodology) and work plan for implementation. - A copy of the company/individual’s latest Audited Financial Statement. - Proof of locality of bidder in the form of municipal bill, lease agreement or letter from ward councillor. - At least five traceable references: provide the names and contact details of any past or present clients you have supplied similar services to over the past 5 years. 14.1. An original valid Tax Clearance Certificate / Valid Tax Compliance Status Pin must be submitted together with the bid. Failure to submit the original and valid Tax Clearance Certificate will result in the invalidation of the bid. Certified copies of the Tax Clearance Certificate will not be acceptable. In bids where Consortia /Joint Ventures/Sub–contractors are involved each party must submit a separate valid Tax Clearance Certificate..
