The subject of the public contract is the provision of rehabilitative reconditioning activities within the framework of health prevention programs for employees of companies hereinafter referred to as organizations with which the contracting authority hereinafter referred to as PZP and the selected supplier will conclude a tripartite agreement on the provision of a financial contribution for the payment of rehabilitative reconditioning stays nRehabilitative reconditioning activities will be implemented in the form of rehabilitative reconditioning stays and will be provided to employees of organizations who are PZP insured persons and who perform work at workplaces with increased physical and mental stress using natural healing sources and or climatic conditions favorable for treatment with the aim of protecting the health of employees Part of the subject of the public contract is also the preparation of a preventive rehabilitation plan for a rehabilitation stay provision of accommodation meals and other activities specified in the procurement documentation and its appendices nRehabilitation rehabilitation stays hereinafter referred to as stays will be focused on respiratory problems and movement problems n2Part of the stay will be procedures applying natural mineral water which has properties suitable for therapeutic use in the sense of 2 paragraph 1 of Act No 164 2001 Coll on natural healing sources sources of natural mineral waters natural healing spas and spa places and on the amendment of some related laws Spa Act as amended hereinafter referred to as the Spa Act favorable for the respiratory tract and locomotor system and supplementary procedures If the supplier does not have a permit to use the resource in accordance with 9 et seq of the Spa Act on medical procedures for which the procurement documentation stipulates the obligation to use natural mineral water from a natural medicinal source these procedures can also be secured using the yield of a natural medicinal source packed in consumer packaging subject to compliance with all other procurement conditions related to the performance of this public contract n3The stays will be realized in a facility meeting the conditions of a natural health spa according to Title VI of the Spa Act and part of the second Decree of the Ministry of Health No 423 2001 Coll which establishes the method and scope of evaluation of natural healing sources and sources of natural mineral waters and other details of their use requirements for the environment and equipment of natural healing spas and the requirements of an expert opinion on the usability of natural healing sources and climatic conditions for therapeutic purposes natural mineral water for the production of natural mineral waters and on the environmental condition of natural healing spas decree on springs and spas n nStays will take place in the following rotations n tturn 4 days stay 4 days 3 nights hereinafter also referred to as 4-day stay n tturn 8 days stay 8 days 7 nights hereinafter also referred to as 8-day stay n tturn 10 days stay 10 days 9 nights hereinafter also as 10-day stay n tturn 15 days stay of 15 days 14 nights hereinafter also referred to as 15-day stay n nNumber of participants in total a maximum of 1 700 participants each year a maximum of 850 participants nNumber of participants in individual stays n t4-day stays a maximum of 180 participants in total a maximum of 90 in each year participants n t8-day a total of maximum 620 participants each year a maximum of 310 participants n t10-day a total of maximum 720 participants each year a maximum of 360 participants n t15-day a total of maximum 180 participants each year a maximum of 90 participants n nMore detailed requirements of the contracting authority relating to the subject of the public contract are set out in the procurement documentation and its appendices n nThis is a repeated public contract while in the previous procurement procedure system number in NEN N00025371 which was divided into 6 parts the contracting party has already awarded parts I II V and VI of the public contract 98332000-9 Spa services Czech Republic 98332000-9 Spa services providing rehabilitative and reconditioning activities as part of health preventive programs
