This is a small-scale public order hereinafter referred to as VZMR for the collection washing cleaning and minor repairs of linen for special veterinary and laboratory operations of the Hlu n Military Veterinary Institute in 2025 operating rooms laboratories for clinical samples food and feed samples of the Hlu n Military Veterinary Institute in 2025 It is necessary to ensure 100 implementation of the prescribed technological procedures for laundry for the aforementioned special operations such as for hospitals The fulfillment will be carried out according to the requirements of the contracting authority based on the framework agreement concluded with the winning participant for the year 2025 The provider will ensure individual partial fulfillment by continuous collection washing cleaning minor repairs and delivery Collection will be carried out once a week The list of required laundry for washing and the expected volume for the year 2025 is given in appendix number 2 Individual partial fulfillments will be carried out on the basis of submitted orders which will be submitted for electronic signature via ET NEN The service provider undertakes to carry out the mentioned works according to the established technological procedures which belong to laundry in the healthcare sector hospital The expected amount of the framework agreement is a maximum of CZK 100 000 including VAT for the year 2025 but this amount may not be reached n nRequirements nThe participant will submit in the form of attachments in the VZMR supplier s attachments n- professional qualification requirements according to 77 of ZVZ 134 2016 paragraph 1 paragraph 2 p a n- extract from the commercial register and the trade register n- decision of the Regional Hygiene Station for the treatment of linen and laundry from medical facilities nFurther filling in the price offer in the table - see appendix no 2 ATTENTION start according to the instructions under the table in appendix no 2 nAs appendix no 3 insert a discussed and possibly amended draft of the framework agreement which is signed by a person authorized to act on behalf of the participant s organization nAs appendix no 4 insert a completed sworn statement of the supplier nIf the order is implemented with the help of a subcontractor we require a valid contract with the subcontractor and the participant to be submitted to the VZMR and also submit the subcontractor s qualification requirements according to 77 Z ZVZ para 1 para 2 p and If it is not documented the sponsor can exclude the participant from the VZMR n nSubmit the required attachments in PDF format The required documents proving the fulfillment of the basic qualification requirements and extracts from the commercial trade register must not be older than 3 months from the date of the announcement of the VZMR 2025 according to the client s requirements nThe place of fulfillment is the Military Veterinary Institute Hlu n Opavsk 29 748 01 Hlu n n 98310000-9 Washing and cleaning Moravian-Silesian Region 98310000-9 Washing and xa0cleaning Washing and xa0cleaning Military Veterinary Institute Hlu n Opavsk 29 748 01 Hlu n
