Must submit a valid Business Permit/License 4. Must submit a Copy of Current Valid Tax Compliance Certificate issued bythe Kenya Revenue Authority 5. Valid NCA Certificate category 8 and above (Building works) 6. Current NCA practicing license 7. Must submit Tender Form duly Completed, Signed and Stamped by TheTenderer in the format provided. 8. Must submit a dully filled, signed and stamped Confidential BusinessQuestionnaire in format provided 9. Must fill certificate of independent tender determination in the formatprovided 10. Must fill Self declaration forms SD 1: - self-declaration that the person / Tenderer is not debarred in the matter of the Public Procurement andAsset disposal Act,2015 11. Must fill Self declaration forms SD 2: - self-declaration that the person /Tenderer will not engage on any corrupt of fraudulent practice 12. Must fill the declaration and commitment to the code of ethics form in the willnot engage in any corrupt or fraudulent practice format provided 13. Must Fill the Supplier Details Form in the Format Provided 14. All pages of the tender documents including the attachments must be duly paginated sequentially in a continuous ascending order from the first page tothe last page. 15. Must attach a valid bid security of Ksh. 130,000.00 (One hundred and thirty thousand shillings only) and the original bid security must be submitted to the Office of the Director, supply chain management services situated at Governor’s administration Block savani Road or provide a mechanism to verify the securities such as QR codes or an online portal before the date and time 15th May,2023 at 12.00 Noon..
