They should also submit a duly signed Power of Attorney. (xiv) A duly signed and stamped self-declaration statement that the firm and its directors had not been debarred from participating in public procurement pursuant to Sec. 115 of the Act & Regulation 91. (xv) Stock List and/or Price List (For Category 2, 3 & 6) will be an added advantage (xvi) Inspection report from Mechanical Department, Certificate of Good Conduct from Police Department and Insurance Certificate against fire and burglary (for Category 4) (xvii) Certificate of Good Conduct from Police Department and Certificate from Communication Commission of Kenya (for Category 3) (xviii) Past Performance (All Category) (provide documentary proofs) with tender notice no.
