Dully filled, signed and stamped self-declaration form SD1 (Public Procurement and Disposal Act non debarment form). 12. Dully filled, signed and stamped self-declaration form SD2 (Anti-corruption pledge). 13. All pages MUST be stamped and serialized/paged. 14.
