Tenders are invited for Renovation of OPD Unit at the Hospital The completed bids are to be submitted in two (2) copies each, one (1) original and one (1) copy. The documents should be in one, sealed envelope clearly marked Quotation with the appropriate project title and lot number at the top right hand corner and addressed to the Medical Director, Federal Neuro- psychiatric Hospital, Dawanau, Kano. ELIGIBILITY REQUIREMENTS Interested, reputable and competent companies with adequate financial resources wishing to carry out any of the aforementioned projects are required to submit the following documents in their quotation, which will be subjected to verification. a. Evidence of Certificate of Incorporation issued by the Corporate Affairs Commission (CAC). b. Evidence of Company’s Income Tax Clearance Certificate (or Personal Income Tax Clearance Certificate of all the Partners in case of Business Name) for the last three (3) years valid till 31st December, 2023. c. Evidence of PENCOM Compliance Certificate valid till 31st December, 2023. d. Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2023. e. Evidence of Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2023. f. Evidence of registration on the National Database of Federal Contractors. Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31st December, 2023 or valid Certificate issued by BPP g. Sworn Affidavit in line with section 16 (e,f) of Public Procurement Act. 2007. h. Company’s Audited Accounts for the last three (3) years 2020, 2021 & 2022 duly acknowledged by the Federal Inland Revenue Service. i. All documents for submission must be transmitted with a covering/forwarding letter under the company’s Letter Head bearing among others, registration number (RC) as issued by the Corporate Affairs Commission (CAC). Contact Address Telephone Number (preferably the Directors of the company) FINANCIAL CAPACITY a. Annual Average Turnover: Minimum of 3m. b. Submission of Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when required. TECHNICAL CAPACITY a. Verifiable documentary evidence of at least three (1) similar job executed (30 years including letters of awards, valuation certificates, job completion certificate and photographs of the projects. b. For Purchase of Vehicles, bid must be accompanied with letter from the Federal Government approved lists of automobile manufacturers/their dealers c. For Purchase of Clinical and Hospital Equipment, bid must be accompanied with letter from Federal Government approved lists of Original Medical Equipment Manufacturers. d. Evidence of key personnel with years of experience and registration with professional bodies: COREN, NSE, CARBON etc. e. Lists of Equipment with Evidence of Ownership or lease agreement. Note: The above listed requirements shall form part of the Bids examination and evaluation criteria, and non-compliance with any of the stated criteria shall result in automatic disqualification of the bidder.
