DOCUMENTS SUBMITTED UPON THE REQUEST OF THE CONTRACTING AUTHORITY ---> The Contracting Authority will request the Contractor whose offer was rated the highest to submit, within a period of not less than 10 days, the following subjective evidence and declarations: 1) JEDZ DECLARATIONS (Single European Procurement Document) – within the scope of Part IV of the JEDZ, the Contractor is limited to completing section Alpha only; 2) INFORMATION FROM THE NATIONAL CRIMINAL REGISTER (KRK) within the scope specified in art. 108 sec. 1 points 1, 2 and 4 of the PZP Act – prepared no earlier than 6 months before its submission; 3) CONTRACTOR’S DECLARATION (prepared according to the template – Appendix No. 8 to the SWZ), including: a) declaration on not belonging to the same capital group (art. 108 sec. 1 item 5 of the PZP Act); b) declaration on the validity of the information contained in the ESPD regarding the grounds for exclusion under art. 108 sec. 1 items 3, 4, 5 and 6 of the PZP Act; c) declaration on not being subject to exclusion on the basis of sanctions provisions (art. 5k of Council Regulation (EU) No 833/2014 and art. 7 sec. 1 of the Act of 13 April 2022 on specific arrangements to counteract support for aggression against Ukraine and to protect national security); 4) LIST OF CONSTRUCTION WORKS ALONG WITH EVIDENCE OF PROPER PERFORMANCE — APPENDIX No. 4; 5) LIST OF PERSONS — APPENDIX No. 5; 6) INFORMATION FROM A BANK OR SKOK POLISH CREDIT UNION CONFIRMING FINANCIAL RESOURCES OR CREDIT CAPACITY. 7) EXCERPT OR INFORMATION FROM THE KRS / CEIDG (or other appropriate register) – in order to confirm the authorization, unless the Ordering Party is unable to download it using free databases. +++ 4. SCOPE OF THE POSSIBILITY OF AMENDING THE AGREEMENT --> The Ordering Party allows the following amendments to the agreement (detailed regulations in the draft agreement):1) amendment to the contract at the Contractor’s request regarding an extension of the order completion deadline; 2) amendment to the contract resulting from the need to: a) perform works not provided for in the statement of planned works (bill of quantities); b) perform works in larger quantities than assumed in the bill of quantities; c) introduce replacement works; d) non-performance of omitted works; 3) amendment to the contract concerning detailing the actual final remuneration. Due to the adopted method of determining remuneration using the as-built cost estimate, it is possible that the amount of the final remuneration will be higher than the amount resulting from the submitted offer and specified in the contract. In such a case, the Ordering Party reserves the right to amend the contract by specifying the actual final remuneration; 4) amendment to a contract concluded for a period longer than 6 months – regarding a change in the amount of remuneration, in the event of a change in the price of materials or costs related to the execution of the order, referred to in Art. 439 of the Act; 5) amendment to the contract regarding a change in the amount of remuneration, in the cases of changes referred to in Art. 436 item 4 letter b) b) of the Act, if these changes will affect the costs of performing the contract by the contractor, 6) in other cases referred to in Art. 455 of the Act. +++ 5.
