All waste generated during the implementation of the subject matter of the contract will be disposed of in a manner that minimizes the environmental impact. The Contractor further declares that it will selectively manage waste generated during the installation of the devices. The Contractor is required to have a current entry in the BDO register, if such an obligation exists under applicable law. 4) Installation works and installed devices will be adapted to the requirements of the Regulation of the Minister of Health of March 26, 2019, which will ensure appropriate premises and technical conditions for providing medical care. 5) The contract is being carried out in accordance with the principle of equal opportunities and non-discrimination, as well as the principle of equal opportunities for women and men. 6) The provisions apply to the medical devices listed in the technical parameters constituting Appendix No. 11 to the Technical Specifications and in the Pricing Form constituting Appendix No. 3. The provisions of Regulation (EU) 2022/1031 of the European Parliament and of the Council of June 23, 2022 shall apply to the procedure. on access of economic operators, goods and services from third countries to the Union public procurement and concessions markets and procedures supporting negotiations on access of EU economic operators, goods and services to the public procurement and concessions markets of third countries (International Procurement Instrument – IPI) and Commission Implementing Regulation (EU) 2025/1197 of 19 June 2025 imposing an IPI measure restricting the access of economic operators and medical devices originating in the People's Republic of China to the Union public procurement market for medical devices in accordance with that Regulation, insofar as this procedure covers medical devices covered by that Regulation. Detailed provisions in this respect can be found in Chapter 5 (V) of the ToR. THE OFFER MUST INCLUDE THE FOLLOWING DECLARATIONS AND DOCUMENTS: 1) JEDZ Declaration - Appendix No. 1 to the ToR 2) Correctly completed Offer Form, prepared according to the template constituting Appendix No. 2 to the ToR; 3) Correctly completed Appendix No. 3 to the Technical Specifications – Pricing Form. 4) Correctly completed Appendix No. 3a – Collective Cost Summary. 5) Contractor’s Declaration regarding the grounds for exclusion – Appendix No. 10 to the Technical Specifications. 6) Declaration of the entity providing the resources regarding the grounds for exclusion – Appendix No. 10a to the Technical Specifications. 7) Correctly completed Appendix No. 11 to the Technical Specifications – Technical parameters of the equipment set. 8) a power of attorney specifying the scope of authorization and signed by the persons authorized to represent the Contractor – if the Contractor applying for the contract is represented by a proxy; 9) a power of attorney to represent Contractors jointly applying for the contract – if applicable. 10) Proof of payment of the deposit ON-SITE INSPECTION The Ordering Party requires the Contractor to conduct an on-site inspection of the site where the investment is planned and parts of the buildings that will be connected to the investment in order to familiarize themselves with the condition of the site and the aforementioned buildings, the conditions of work implementation, and any potential organizational and logistical difficulties and conditions resulting from carrying out construction works in active facilities, in order to incorporate the above circumstances into the offer price. The authorized contact persons are Mr. Zbigniew Gajda – Investment Coordinator, Mr. Grzegorz Dźwierzyński – Construction Inspector, and Mr. Maciej Przystupa – Electrical Specialist. The on-site inspection will take place on two dates: September 30, 2026 and October 8, 2026 at 10:00 AM. Interested Contractors are asked to report to room no. 103 in Building H of the hospital on the selected date. Confirmation of the on-site visit will be confirmation of attendance on the list provided by the Ordering Party on the day of the visit. Pursuant to Article 131, Section 2, Item 1) of the Public Procurement Law, the Ordering Party requires the Contractor to submit a bid after the on-site visit. The Contracting Authority hereby informs that it will reject the Contractor's offer pursuant to Article 226, Section 1, Item 18 of the Public Procurement Law if the Contractor submits the offer without conducting an on-site inspection.
