The Provincial Specialist Hospital in Biała Podlaska in Poland from Europe region has released this tender for The subject of the order is the supply of CSP implantation kits, CRT-D and CRT-P resynchronization procedure kits, products used in the Electrophysiology and Cardiac Electrotherapy Laboratory (for repairs), non-compliant balloons (controlled compliance), low-profile semi-compliant balloons, semi-compliant paclitaxel-eluting balloons with a hydrophilic coating, cutting balloons with two wires participating in the dilation process, RX non-compliant paclitaxel-eluting balloons with a hydrophilic coating, microcatheters, RX balloon catheters for in-stent restenosis, an assortment of high-risk procedures supporting the left ventricle, torquers for angioplasty guidewire manipulation, PTFE-coated diagnostic guidewires for coronary imaging, intravascular lithotripsy systems, low-profile abdominal aortic stent grafts preserving the natural aortic bifurcation with the possibility of suprarenal and infrarenal fixation. Low-profile abdominal aortic stent grafts based on polymer sealing technology, based on polymer sealing technology that preserve the natural aortic bifurcation, iliac stent grafts with a low-profile delivery system, diagnostic angiographic catheters for vascular examinations for selective vessel contrast, short reinforced sheaths, plates for bone fracture fixation, screws, knee joint prostheses, hip joint endoprostheses, including revision ones, and devices for orthopedic surgery, divided into 21 lots. The Contracting Authority – pursuant to Art. 139 of the Public Procurement Law – hereby informs that it will first examine and evaluate the offers in this public procurement procedure, and then select the contractor whose offer was rated the highest in terms of the absence of grounds for exclusion and compliance with the conditions for participation in the procedure. DOCUMENTS TO BE SUBMITTED WITH THE OFFER 1. The offer consists of: 1) a completed offer form, in accordance with Appendix No. 1 to the ToR; 2) a completed assortment and pricing form, in accordance with Appendix No. 2 to the Technical Specifications; 3) for parts No. 1 and 2: completed Appendix No. 4 and 4A to the Technical Specifications (technical parameters). 2. Along with the offer, the Contractor is required to submit: 1) an extract or information from the National Court Register, the Central Registration and Information on Business, or another appropriate register – to confirm that the person acting on behalf of the Contractor is authorized to represent it. The Contractor is not obliged to submit these documents if the Ordering Party can obtain them through free and publicly available databases, provided the Contractor has provided the data enabling access to these documents; 2) if a person whose authorization to represent the Contractor does not result from the documents referred to in the preceding point acts on behalf of the Contractor:a power of attorney or other document confirming the authorization to represent the contractor; 3) a power of attorney to represent Contractors jointly applying for the contract award – if applicable; 4) in the case of a bid security in a form other than cash – the original of the bid security document (guarantee or surety) – if applicable. 5) The declaration and documents indicated in chapter VI A to the extent specified therein as to be submitted with the offer.
