DELIVERY OF ONCOLOGICAL DRUGS FOR THE PROVINCIAL SPECIALIZED HOSPITAL NAMED AFTER THE BLESSED VIRGIN MARY IN CZ STOCHOWA

The Provincial Specialist Hospital named after the Blessed Virgin Mary in Poland from Europe region has released this tender for DESCRIPTION OF THE SUBJECT OF THE ORDER 1 The subject of the order is SUPPLY OF ONCOLOGICAL DRUGS for the Provincial Specialist Hospital named after the Blessed Virgin Mary in Cz stochowa The subject of the order comprises 15 lots 2 The detailed scope of the order is specified in Appendix No 2 to the ToR the assortment and pricing form Other conditions regarding the performance of the order are specified in the draft contract provisions PPU constituting Appendix No 6 to the ToR 3 All requirements specified in the documents indicated above constitute minimum requirements and their fulfillment is mandatory Failure to meet the above-mentioned minimum requirements will result in rejection of the offer as inconsistent with the terms of the order pursuant to art 226 sec 1 item 5 of the Public Procurement Law 4 The Ordering Party allows the submission of partial offers A partial offer is understood as individual lots specified in Appendix No 2 to the ToR The Contractor may submit an offer for any lots or the entire subject of the order 5 When describing the subject of the contract by reference to standards technical assessments technical specifications and technical reference systems referred to in art 101 sec 1 item 2 and sec 3 of the Public Procurement Law the Ordering Party allows solutions equivalent to those described If the Ordering Party indicates trademarks patents or origin in the order documents the Ordering Party allows the use of equivalent solutions 6 CPV code 33652000-5 7 The Ordering Party agrees to offer preparations interchangeably i e tablets coated tablets drogs capsules hard capsules soft capsules vials ampoules pre-filled syringes injectors bottles flasks 8 The Ordering Party agrees to change the size of packaging of liquids syrups ointments and creams with packaging recalculation based on the appropriate unit of measurement i e mg ml 9 The Ordering Party allows the Contractor to delete the subject of the order from a given Lot in the event of termination of production or withdrawal of the subject of the order from the market and to include a note - termination of production withdrawal of the product provided that the offer is accompanied by appropriate documents confirming this fact 10 The Ordering Party consents to the valuation of products available with a one-time permit from the Minister of Health in a situation where only such products are currently available on the market INFORMATION ABOUT THE SUBJECT EVIDENCE 1 In order to confirm the compliance of the offered supplies with the requirements specified in the description of the subject of the order related to the execution of the order the Ordering Party requests the submission of the following evidence together with the offer a a declaration by the Contractor that the offered assortment has all current documents required by Polish law based on which it may be introduced into circulation and used in healthcare facilities in the territory of the Republic of Poland in accordance with the Act of 6 September 2001 Pharmaceutical Law and other applicable legal provisions within the scope of the offered subject of the contract and that the offered oncological drugs are fully registered consistent with the original drug in accordance with the template included in Appendix No 9 to the ToR All required up-to-date documents including Material Safety Data Sheets will be submitted within the specified timeframe at each request of the Ordering Party during the term of the concluded contract within no more than 3 days of such request this applies to offered products classified as pharmaceutical products 2 If the Contractor has not submitted the evidence in question or the evidence submitted is incomplete the Ordering Party will request its submission or supplementation within the specified timeframe Art 107 2 of the Public Procurement Law 3 The Ordering Party hereby informs that despite the anticipated request it will not request the submission or supplementation of the evidence in question if the evidence in question serves to confirm compliance with the characteristics or criteria specified in the description of the offer evaluation criteria or despite the submission of the evidence in question the offer is subject to rejection or there are grounds for invalidating the procedure 4 The Ordering Party may request explanations from Contractors regarding the content of the evidence in question 5 During the examination and evaluation of offers the Ordering Party may request explanations from Contractors regarding the content of submitted offers and the evidence in question or other documents or declarations submitted Article 223 section 1 first sentence of the Public Procurement Law 6 The Ordering Party will accept equivalent evidence in question if it confirms that the offered supplies meet the requirements specified by the Ordering Party 7 The evidence in question subjective evidence and other documents or declarations drawn up in a foreign language shall be submitted together with a translation into Polish DEPOSIT REQUIREMENTS A contractor entering the proceedings is required to submit a deposit before the deadline for submission of offers The required deposit amount is PLN 412 672 00 The deposit amount for individual lots is specified in the table in Chapter X of the ToR INFORMATION ON THE CONDITIONS FOR PARTICIPATION IN THE PROCEDURE 1 The award of the contract may be applied for by Contractors who meet the conditions for participation in the procedure regarding the authorization to conduct specific business or professional activities provided that this results from separate provisions This condition will be met if the Contractor proves that it has a valid and current permit license of the Chief Pharmaceutical Inspector for trading in pharmaceuticals admitted to trading or a valid and current permit to operate a pharmaceutical wholesaler in accordance with the Pharmaceutical Law Act and other applicable legal provisions in this respect in the case of a Contractor with its registered office or place of residence outside the territory of the Republic of Poland an equivalent document In the event thatif no license or permit is required to conduct business activity the Contractor is obliged to attach an appropriate declaration with tender notice no. 665657-2026 published on 07 Oct 2026.
