Delivery of reagents for biochemical determinations with analyzer rental for a period of 48 months

The subject of the order is: _ supply of reagents for immunochemical assays with the rental of an analyzer for a period of 48 months. 2. A detailed description of the subject of the order is included in the price form and the technical specifications of the analyzer, which constitute Annexes 1A and 4 to the Technical Specifications. 1.1. In order to confirm the compliance of the offered supplies, the ordering party requests the submission of the following evidence together with the offer: 1) 1) A declaration that the offered assortment has been admitted and introduced to the market in accordance with the provisions of the Act of 7 April 2022 on medical devices (Journal of Laws of 2022, item 974) and has valid declarations of conformity and certificates of a notified body and will be made available at each request of the ordering party after signing the contract - the content of the declaration is entered in the offer form constituting Appendix No. 1 to the SWZ 2) Analyzer catalog card confirming the parameters indicated in Appendix No. 4 to the SWZ 3) Leaflets for the offered reagents 4) Declaration of conformity of the analyzer - a document issued by the manufacturer of the product or its authorized representative. 5) Manufacturers catalog cards and/or analyzer operating instructions confirming the assessed parameters (offer evaluation criteria: K2, K3, K4, K5 described in point XV of the Technical Specifications). 2. Applies to point VIII.1 item 1) The Ordering Party does not plan to supplement the evidence indicated in point VIII.1 item 1). Offers that do not contain the aforementioned documents will be rejected pursuant to art. 226 section 5 of the Public Procurement Law. 3. Applies to point VIII.1 items 2), 3), 4) If the Contractor does not submit the documents indicated in point VIII.1 items 2), 3), 4) with the offer or the submitted evidence is incomplete, the Ordering Party will request that they be submitted or supplemented within the specified deadline. 4. Applies to point VIII.1 item 5) The Ordering Party does not plan to supplement the documents indicated in point VIII.1 item 5) confirming the assessment parameters. In the absence of such documents, the Contractor will receive 0 points. 5. The evidence in question prepared in a foreign language must be submitted together with a translation into Polish. 6. The Ordering Party requires that the evidence in question be prepared in electronic form and bear a qualified signature by the Contractor. 7. If the evidence in question was prepared in electronic form and bear a qualified signature by its issuer (authorized entity), it must be submitted in this form. 8. If the evidence in question was issued in paper form, a digital copy of the document must be submitted bearing a qualified signature by the Contractor. 9. The Ordering Party requires that the evidence in question confirm the factual circumstances as of the date of bid submission. 10.
