The Tarn w Academy in Poland from Europe region has released this tender for 1 The subject of the order is the supply of medical equipment for the Tarn w University The subject of the order has been divided into 20 lots and includes Part 1 Trainers Part 2 Dressings Part 3 Bandages and materials for compression therapy Part 4 Fluids Part 5 Needles syringes and cannulas Part 6 Infusion sets and accessories Part 7 Needles and tubes for blood collection Part 8 Other disposable and laboratory materials Part 9 Catheterization and urological materials Part 10 Disinfectants Part 11 Personal protective equipment and hygiene materials Part 12 Ostomy materials Part 13 Small diagnostic equipment Part 14 Consumables for medical devices Part 15 Materials for CPR oxygen therapy and airway management Part 16 Materials for enteral nutrition Part 17 Products for diabetics Part 18 Surgical materials Part 19 Electrotherapy Part 20 Overbed panels 2 The ordering party does not limit The number of order lots submitted by one Contractor One Contractor may submit a bid for the entire order i e 20 lots or any number of lots 3 Bids for partial lots are not permitted 4 The provisions of this ToR apply to all parts of the procedure unless otherwise stated 5 The Ordering Party requires the submission of new products of appropriate quality free from physical and legal defects and meeting legally required standards and certificates as well as approved for sale and use in Poland in accordance with applicable law meeting quality and technical standards resulting from the function and intended use 6 The subject of the order must meet all requirements for a safe product in accordance with Regulation EU 2023 988 of the European Parliament and of the Council of 10 May 2023 on general product safety amending Regulation EU No 1025 2012 of the European Parliament and of the Council and Directive EU 2020 1828 of the European Parliament and of the Council and repealing Directive 2001 95 EC of the European Parliament and of the Council and Council Directive 87 357 EEC of 10 May 2023 OJ L 135 p 1 to the extent that the regulation applies The Contractor is obliged to deliver a brand new subject of the order in particular not repaired not remanufactured not exhibition not previously used from current production and a legal distribution channel functional complete in particular with all components parts materials and compliant with the DSC 7 Wherever the Technical Specifications and their annexes contain terms indicating trademarks patents or origin sources or a specific process that characterizes the products delivered by a specific contractor such an indication is accompanied by the words or equivalent the Ordering Party allows the Contractors to offer equivalent products which will be assessed based on the equivalence assessment criteria described in Appendix No 2 to the ToR a The Contractor is obligated to demonstrate equivalence at the time of submitting the offer the Contractor selects the appropriate option in point 8 of the offer form Appendix No 1 to the ToR b If the Contractor proposes equivalent solutions the Contractor shall submit a list of equivalent solutions an appendix to the offer form along with the offer and attach evidence of equivalence Failure to submit a list of equivalent solutions and evidence of equivalence with the offer will result in rejection of the offer c If the Contractor fails to select any option in point 8 of the offer form the Ordering Party will assume that during the contract execution phase the Contractor will use the solutions described in the ToR and the appendices thereto 8 The Ordering Party specifies the equivalence criteria i e the required parameters properties and functionalities of the offered products based on which it will assess equivalence The equivalence criteria are described in Appendix No 2 Description of the Subject Matter of the Order 9 The Contractor is obligated to deliver the products to the Ordering Party using the Contractor s own transport or through a courier company that meets the required quality standards for the transported goods at its own expense and risk 10 The subject of the contract must be delivered to the Ordering Party in the original factory packaging The place of delivery of the subject of the contract as well as the place of implementation and installation is Akademia Tarnowska ul Mickiewicza 8 33-100 Tarn w 11 The subject of the contract must be delivered in packaging bearing the name serial number and expiration date if applicable 12 The expiration date for the subject of the contract is specified in the offer form Appendix 1 to the ToR for the individual items concerned 10 The subject of the contract must be delivered to the Ordering Party in its original factory packaging The place of delivery of the subject of the contract as well as implementation and installation is Akademia Tarnowska ul Mickiewicza 8 33-100 Tarn w 11 The subject of the contract must be delivered in packaging marked with the name serial number and expiration date if applicable 12 The expiration date for the subject of the contract
