The Independent Public Healthcare Facility, Children and Youth Treatment Center (hereinafter referred to as the Client), intends to build a new hospital headquarters located in Zielona Góra. The new facility will include a comprehensive psychiatric treatment center for children and youth, along with school and administrative facilities, a day care center, and permanent residence wards. The scope of work should be adapted to the Clients requirements presented in the Functional-Utility Program. The Program specifies the Clients requirements and expectations for the investment in question. The Program precisely specifies the scope, quality, and requirements for the equipment to be delivered and installed as part of the Investment. As part of the project, the Contractor should detail and expand the functional layout defined in the Program in a manner consistent with the aforementioned regulations and the conditions specified for the individual equipment to be installed. The Contractors actions and the results of their work must comply with the applicable legal order. Program The Functional-Utility Plan is used to estimate the planned costs of design and construction works and to prepare the offer, particularly in terms of calculating the offer price. The following activities must be performed as part of the planned project: 1. Pre-design work, including: - preparing a map for design purposes, - preparing documentation of ground investigations, a geotechnical opinion, and a geotechnical design, - preparing and obtaining a decision on approval of the geological works project to determine the geological and engineering conditions, - submitting a notification of geological works, - performing geological works, - preparing and obtaining a decision on approval of the geological and engineering documentation, - and other research or measurements necessary to carry out the planned project; - preparing a dendrological report together with obtaining the necessary opinions regarding tree felling, - submitting a water-legal notification for excavation drainage (if required). 2. Preparing a construction design along with the necessary approvals in the form of: - a land development plan, - an architectural and construction design, - annexes to the construction design, - a technical design. 3. Obtaining a building permit – at the discretion of the Contractors; 4. Notification of the intention to perform construction works that do not require a building permit, such as networks and other works necessary to carry out the planned project; 5. Preparation of detailed designs to the extent necessary for the implementation of the investment; 6.Obtaining a decision permitting the cutting down of trees and shrubs (administrative fees and fees resulting from the decision permitting the cutting down of trees are the responsibility of the Ordering Party) 7. preparation of a compensatory planting design together with obtaining the necessary opinions and approvals (assuming the possibility of planting on the area of ​​cadastral plots no. 50/10, 50/11, 50/12, 50/13, 50/20, 50/22, 50/23, 50/24, district 16, Zielona Góra, (the implementation of compensatory planting is the responsibility of the Ordering Party) 8. Execution of the entire investment in accordance with the building permit and the developed designs; 9. Delivery and assembly of permanently installed equipment; 10.
