along with other activities related to the management and settlement of the project entitled Support for Oncological Rehabilitation at the West Pomeranian Oncology Center 2 The subject of the contract is the provision of investor supervision services by a multi-disciplinary Investor Supervision Inspector Team along with formal legal reporting and project settlement services in accordance with the funding agreement during the implementation of the following specific tasks Task No 1 Construction of the Oncological Rehabilitation Center building including land development Design and build construction of a new three-story building with an underground garage intended for the Oncological Rehabilitation Center Task No 2 Supply of rehabilitation equipment and medical and non-medical supplies for the Oncology Rehabilitation Center a purchase delivery and installation of medical equipment including diagnostic therapeutic and rehabilitation equipment for all types of procedures therapies and rehabilitation performed by the Oncology Rehabilitation Center b purchase delivery and installation of all other equipment including administrative office and social equipment for patients and staff necessary for the proper functioning of the unit 3 Implementation of the subject matter of the contract consists of the following stages 1 stage I from the moment of conclusion of the agreement between the Ordering Party and the Supervision Inspector to the moment of conclusion of the agreement between the Ordering Party and the Contractor for the design documentation and construction works under Task 1 stage I includes among other things support in the process of preparing the Technical Specifications SWZ and support in the process of conducting a public procurement procedure to select a Contractor for the design documentation and construction works as part of the implementation of specific tasks assumed estimated implementation of stage I up to 4 months from the date of concluding the agreement with the Supervision Inspector deadline for developing documents being the basis for commencing the proceedings Terms of Reference with annexes up to 1 month from the date of concluding the agreement with the Supervision Inspector 2 stage II from the moment of concluding the agreement between the Ordering Party and the Contractor for the design documentation and construction works as part of task 1 until the moment of obtaining a final building permit decision after which it will be possible to commence construction works stage II includes among others supervision of the development of the building design assumed implementation up to 5 months from the date of concluding the agreement with the Contractor for the design documentation and construction works 3 stage III from the moment of obtaining the building permit decision until the moment of handing over the facility for use including the final acceptance of the facility preparation of the application for an occupancy permit obtaining the occupancy permit and necessary documentation preparation of the building for use stage III includes among others supervision over the preparation of detailed documentation and the implementation of construction and installation works development of a delivery schedule and principles for their coordination assumed implementation of stage III up to 23 months 4 stage IV from handover of the facility for use to the final settlement of the subsidy including the submission of relevant documents reports settlements list of fixed assets including the submission of the final report within 30 days of handover of the facility for use assumed implementation of stage IV no later than December 31 2029 4 In order to implement the investment task Support
