Electronic processing of real estate tax for legal entities - 4th maturity level, 4. Electronic processing of agricultural tax for individuals - 4th maturity level, 5. Electronic processing of tax on means of transport - 4th maturity level, 6. Electronic processing of municipal waste management liabilities - 5th maturity level, 7. Submitting an application for an extract and an outline from the local spatial development plan - 4th maturity level, 8. Submitting an application for a certificate of plot designation in the local spatial development plan - 4th maturity level, 9. Application for an extract and an extract from the study / POG - 4th maturity level, 10. Application for a certificate from the study / POG - 4th maturity level, 11. Application for determining the serial number of a building - 4th level maturity, 12. Application for a permit for the location/reconstruction of an exit - maturity level 5, 13. Application for a decision on the location of the route of the planned cable connection - maturity level 5, 14. Application for a decision on the temporary occupation of a road lane – maturity level 5, 15. Application for a decision on the placement of devices in a road lane – maturity level 5. 2) System user training and cybersecurity training. 3) Delivery of an info kiosk. 3. The subject of the contract must be implemented for all users, including in particular requirements regarding accessibility for disabled persons. 4. Subjective evidence 1) In order to confirm the compliance of the offered products with the Ordering Party's requirements within the scope indicated in the description of the subject of the contract, pursuant to art. 106 sec. 1, in conjunction with art. 107 sec. 1 of the Public Procurement Law, the Ordering Party requests that the offer be accompanied by the subjective evidence confirming the compliance of the solutions offered by the Contractor with the Ordering Party's requirements, i.e. the provided software sample. 2) The sample constitutes the subjective evidence used to confirm the compliance of the offered solution with the OPZ requirements. Failure to demonstrate, by means of a sample, that the requirements specified by the Ordering Party are met will be treated as non-compliance of the offer with the terms of the contract. 3) Detailed requirements regarding the content of the sample and the method of testing it are included in the Appendix to the Technical Specifications – Sample Testing. 4) The Ordering Party, pursuant to art. 65 sec. 1 item 4 of the Public Procurement Act, waives the requirement to use electronic means of communication with regard to part of the offer – the software sample, which constitutes the subject evidence. 5) In connection with the above, pursuant to art. 65 sec. 2 of the Public Procurement Act, the Ordering Party requires that the above-mentioned sample containing two portable drives, prepared in accordance with the guidelines contained in the Appendix to the Technical Specifications – Sample Testing, be delivered: a. via a postal operator within the meaning of the Act of 23 November 2012 – Postal Law (consolidated text: Journal of Laws of 2025, item 366, as amended) or b. in person or c. by a messenger, to the Ordering Party’s address: Municipal Office in Sompolno, ul. 11 Listopada 15, 62-610 Sompolno by the deadline for submitting offers (i.e. by 28/09/2026). 10:00) on the Ordering Party's purchasing platform (before the deadline for submission of offers). 6) The sample must be submitted in a tightly closed package, preventing its tampering without tearing, marked with the name and address of the Contractor, the name and address of the Ordering Party, the procedure reference number – RZ.271.6.2026 and the annotation: "Software sample, do not open before September 28, 2026, 10:15 AM." 7) Notwithstanding the above, the remaining documents comprising the offer, in accordance with Art. 65 sec. 3 of the Public Procurement Law, shall be submitted by the Contractor in electronic form on the Ordering Party's purchasing platform. 8) The date of offer submission shall be understood as the date its receipt on the Platform, while the date of sample submission shall be understood as the date and time of its receipt at the Ordering Party's registered office, to the address indicated above in point 4). 9) The Ordering Party will return the submitted samples in accordance with the principles arising from Art. 77 of the Public Procurement Law. 10) The Ordering Party does not intend to cover the costs of sample preparation.
