ToR [regarding the grounds for exclusion under Article 5k) of Council Regulation (EU) No 833/2014 of 31 July 2014, as amended by Regulation 2025/2033 (OJ L, 2025/2033, 23.10.2025)] – the declaration should be submitted in accordance with the template constituting Annex No. 4 to the ToR. These declarations constitute evidence confirming the absence of grounds for exclusion and compliance with the conditions for participation in the procedure, as of the date of submission of offers, temporarily replacing the required subjective means of evidence indicated in Chapter IV, section 2, point 2.1. SWZ, and in the case of submitting a certificate – a declaration to the extent to which the certificate does not demonstrate compliance with these conditions - Part II, section A of the ESPD (line concerning information whether the contractor is entered on the official list of approved contractors or holds an equivalent certificate). 3. The declaration referred to in Chapter IV, section 1, item 1.2. SWZ, letters a) – b) should be submitted in the form of a European Single Procurement Document, drawn up in accordance with the standard form specified in the European Commission Implementing Regulation issued on the basis of art. 59 paragraph 2 of Directive 2014/24/EU, hereinafter referred to as the "Single Document". The Contractor is obliged to submit the Single Document to the extent indicated in Annex No. 3 to the SWZ. The Contracting Authority hereby informs that the Instructions for completing the Single Document are available on the website of the Public Procurement Office. 4. The template of the Single Document submitted by the Contracting Authority is compliant with Commission Implementing Regulation (EU) 2016/7 of 5 January 2016 establishing a standard form for the European Single Procurement Document. The Contractor is obliged to complete the Single Document to such an extent that the Contracting Authority is able to verify whether the Contractor meets the conditions for participation in the procedure and is not subject to exclusion to the extent described in these ToR. 5. The declarations referred to in Chapter IV, section 1, item 1.2 of the ToR are submitted separately by:1) The Contractor/each of the Contractors jointly applying for the contract.
