The Ordering Party requests that the Contractor indicate in its offer the parts of the order that it will subcontract and provide the names of any subcontractors, if already known. 8. Delivery time of the assortment: up to 45 business days, counted from the date of conclusion of the implementation agreement or acceptance of the demand. 9. The offer is valid until January 20, 2027. 10. The Ordering Party will make changes to the contract in accordance with Chapters XIX and XIX a of the SWZ. 11. In accordance with Article 13 paragraphs 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (i.e. OJ EU L 119 of 4.05.2016, p. 1), hereinafter referred to as "GDPR", the Ordering Party informs that: 1) the controller of your personal data is the Commander of the Warsaw Police; 2) supervision over the proper processing of personal data is exercised by the personal data protection inspector: address: ul. Nowolipie 2, 00-150 Warsaw; e-mail address: iod@ksp.policja.gov.pl 3) Your personal data will be processed on the basis of Article 6 paragraph 1 letter c of the GDPR for the purpose of the proceedings conducted as an open tender in order to conclude a framework agreement for the supply of LAN radio controls for VHF and UHF radiotelephones and dispatch consoles (Procedure number: WZP-2588/26/144/Ł). 4) the recipients of your personal data will be persons or entities to which the documentation of the proceedings will be made available on the basis of Article 18 and Article 74 sec. 1 of the Act; 5) your personal data will be stored in accordance with Art. 78 of the Act for a period of 4 years from the date of completion of the contract award procedure, and if the duration of the contract exceeds 4 years, the storage period covers the entire duration of the contract; 6) the obligation to provide personal data concerning you directly is a statutory requirement specified in the provisions of the Act, related to participation in the public procurement procedure; the consequences of failure to provide certain data result from the Act; 7) with respect to your personal data, decisions will not be made in an automated manner, pursuant to Art. 22 of the GDPR; 8) you have: a) under Art. 15 of the GDPR, the right to access personal data concerning you; b) under Art. 16 of the GDPR, the right to rectify your personal data*; c) under Art. 18 of the GDPR, the right to request the controller to restrict the processing of personal data, subject to the cases referred to in Art. 18 sec. 2 of the GDPR**; d) the right to lodge a complaint with the President of the Personal Data Protection Office if you believe that the processing of your personal data violates the provisions of the GDPR; 9) you are not entitled to: a) in connection with Article 17 paragraph 3 letters b, d or e of the GDPR, the right to delete personal data; b) the right to transfer personal data, referred to in Article 20 of the GDPR; c) under Article 21 of the GDPR, the right to object to the processing of personal data, as the legal basis for the processing of your personal data is Article 6 paragraph 1 letters b, c and f of the GDPR. *Explanation: exercising the right to rectification cannot result in a change in the outcome of the public procurement procedure or a change in the provisions of the contract to an extent inconsistent with the Act, and cannot violate the integrity of the protocol and its annexes. **Explanation: the right to restrict processing does not apply to storage, to ensure the use of legal remedies or to protect the rights of another natural or legal person, or for important reasons of public interest of the European Union or a Member State. 12. The Ordering Party does not require the Contractor to demonstrate compliance with the conditions specified in Article 112 paragraph 2 of the Act. 13. The award of the contract in question may be applied for by Contractors who are not subject to exclusion under Article 108, paragraph 1 of the Act and art. 109 sec. 1 points 1-10, of the Act and art. 7 sec. 1 of the Act of 13 April 2022 on special solutions for counteracting support for aggression against Ukraine and serving to protect national security (Journal of Laws of 2025, item 514) and art. 5k of Council Regulation (EU) No 833/2014 of 31 July 2014 concerning restrictive measures in view of Russia's actions destabilizing the situation in Ukraine (OJ EU No L 229, 31.7.2014, p. 1), as amended, hereinafter referred to as: Regulation 833/2014, in the version set out in Council Regulation (EU) 2022/576 amending Regulation (EU) No 833/2014 concerning restrictive measures in view of Russia's actions destabilizing the situation in Ukraine (OJ EU No L111, 8.4.2022, p. 1) and as amended by Council Regulation (EU) 2025/2033 of 23 October 2025 amending Regulation (EU) No 833/2014 concerning restrictive measures in view of Russia's actions destabilizing the situation in Ukraine the situation in Ukraine. with tender notice no. 659026-2026 published on 25 Sep 2026.
