The Municipal Social Welfare Center in Poland from Europe region has released this tender for 1. DESCRIPTION OF THE SUBJECT OF THE ORDER 1.1. The subject of the order includes the provision of care services and specialized care services. 1.2. A detailed description of the subject of the order constitutes Appendix No. 2 to the ToR. 1.3. The order has been divided into two (2) lots: a) part 1 – Provision of care services; b) part 2 – Provision of specialized care services. 2. CONTRACT COMPLETION DATE. The contract completion date for all lots is: from 1 January 2027 to 31 December 2027. 3. CONTRACTS REFERRED TO IN ARTICLE 214 SECTION 1 ITEM 7) OF THE PPL Act The Ordering Party does not envisage awarding contracts on the basis of art. 214 sec. 1 item 7) of the PPL Act, consisting in the repetition of similar services. 4. CONTRACTORS JOINTLY COMPETING FOR A PUBLIC CONTRACT 4.1. Contractors may jointly compete for a public contract. In such a case, Contractors acting jointly are required to appoint a proxy to represent them in the procedure or to represent them in the procedure and conclude an agreement regarding the public procurement in question. All correspondence will be conducted by the Ordering Party exclusively with the proxy. 4.2. The condition regarding the authorization to conduct specific business or professional activities, referred to in art. 112 sec. 2 item 2) of the Public Procurement Law (if formulated), will be met if at least one of the Contractors jointly competing for the contract holds authorizations to conduct specific business or professional activities and will perform the services for which these authorizations are required. 4.3. With respect to the conditions concerning education, professional qualifications or experience, Contractors jointly applying for the award of a contract may rely on the capabilities of those Contractors who will perform the services for which these capabilities are required. 4.4. In the case referred to in Chapter I, section 4, point 4.2. (if formulated) and point 4.3. of the ToR, Contractors jointly applying for the award of a contract shall attach to their offer a declaration specifying which services will be performed by individual Contractors. 5. PROTECTION OF PERSONAL DATA COLLECTED BY THE CONTRACTING AUTHORITY DURING THE PROCEDURE A detailed description is included in Chapter I, section 17. of the ToR. 6. REQUIREMENTS REGARDING THE PROPER PERFORMANCE SECURITY Before concluding the contract, the Contractor is obligated to provide a performance security in the amount of 5% of the total price specified in the offer. The contractor must provide security separately for each lot for which their bid is deemed the most advantageous.
