The Fryderyk Chopin University Clinical Hospital in Rzesz w in Poland from Europe region has released this tender for 1 The subject of the order is the purchase and successive delivery of surgical threads and tissue glue specified in Lots No 1 44 for the Fryderyk Chopin University Clinical Hospital in Rzesz w including Lots No 1 33 and 35 - 44 surgical threads Lot No 34 tissue glue 2 A detailed description of the subject of the order and the Ordering Party s requirements are specified in the Price and Offer Forms constituting Appendix No 1 to the ToR for individual Lots which are an integral part of the ToR 3 The Ordering Party requires that the offered items of the order 3 1 meet the requirements contained in the Price and Offer Forms 3 2 in Lots No 1 44 were medical devices compliant with the Act of 7 April 2022 on Medical Devices Journal of Laws of 2024 item 1620 as amended and with the applicable provisions of European Union law in particular Regulation EU 2017 745 of the European Parliament and of the Council MDR to the extent appropriate for a given product including those concerning placing on the market and use conformity assessment CE marking labelling and instructions for use 4 The Ordering Party does not allow the submission of equivalent offers 5 The scope of the order includes the delivery of the items of the order in the quantities specified in the Price and Offer Forms for a period of 24 months from the date of conclusion of the contract for Lots No 1 44 6 The subject of the order within the scope of Lots No 1 to No 44 will be implemented in the form of successive deliveries over a period of 24 months from the date of conclusion of the contract 7 If the entire subject of the order is not completed within the contract period in relation to Lots No 1 to No 44 the Ordering Party reserves the right to extend the contract period The contract term will be amended by a written amendment signed by the Parties The Ordering Party will inform the Contractor of its intention to extend the contract term before the contract period expires 8 Deadline for submitting a request for clarification of the content of the Technical Specifications October 20 2026 with tender notice no. 670296-2026 published on 07 Oct 2026.
