The contract scope includes: 1) development of a multi-disciplinary programmatic and spatial concept; 2) preparation of complete design documentation, including a conservation work program, 3) obtaining the required approvals, opinions, resolutions and administrative decisions, including a building permit, a permit from the Lubusz Voivodeship Conservator of Monuments and other documents required by law, 4) execution of construction works together with installations, 5) execution of conservation works, if they result from a decision of the Lubusz Voivodeship Conservator of Monuments, 6) execution of land development, including the execution of all necessary connections and reconstruction of technical infrastructure, 7) delivery, assembly and start-up of only permanent, built-in, installation and technological equipment and devices necessary for the proper functioning of the facilities, provided they are indicated in the FUP or result from the approved design documentation (the purchase of movable equipment, movable furniture, medical equipment and medical devices is not the subject of the contract), 8) conducting tests, measurements, start-ups and staff training, 9) preparation of as-built documentation, 10) notifying the relevant authorities of the completion of construction works and obtaining an occupancy permit. The Ordering Party envisages the procurements referred to in Article 214, paragraph 1, item 7 of the Public Procurement Law. Within three years of the date of award of the basic procurement, the Ordering Party envisages the possibility of awarding a procurement consisting in the repetition of similar services or construction works, consistent with the subject matter of the basic procurement and constituting no more than PLN 30,000,000.00 net (in words: thirty million zloty). The condition for awarding a procurement consisting in the repetition of similar procurements is the fact that:- the existing contractor will ensure a standard of performance of the new contract no lower than that of the basic contract; - the existing contractor will accept the material terms of the existing contract; - the Ordering Party will conduct negotiations with the existing contractor under the contract award procedure specified in Article 213 et seq. of the Public Procurement Law, as a result of which, in particular, the remuneration and deadline for the completion of such contract will be agreed. The procedure will be conducted in accordance with the "Do No Significant Harm" (DNSH) principle, referred to in Article 17 of Regulation (EU) 2020/852 of the European Parliament and of the Council, and in accordance with Article 5 paragraph 2 of Regulation (EU) 2021/241 of the European Parliament and of the Council establishing a Recovery and Resilience Facility. The contractor is obliged to prepare design documentation and carry out the construction works in a manner that ensures no significant negative impact of the investment on the environmental objectives specified in Article 9 of Regulation (EU) 2020/852. The State Treasury - Minister of Health granted a subsidy for the subject of the contract under contract no.
