Supply of dressing materials, medical devices and ophthalmological accessories for the USK-2 PUM Clinics in Szczecin

The UNIVERSITY CLINICAL HOSPITAL No. 2 PUM IN SZCZECIN in Poland from Europe region has released this tender for 1. The subject of the order is the supply of dressing materials, medical devices, and ophthalmic accessories for the USK-2 PUM Clinics in Szczecin. 2. Whenever the term "products" is used hereinafter, this term should be understood as the products indicated in Appendix 2 (FCJ) to the SWZ; 3. The description of the subject of the order, detailed requirements, and estimated quantities for the products are included in the unit price form (Appendix 2 to the SWZ, hereinafter also referred to as FCJ). This form must be submitted together with the offer form (Annex 1 to the Technical Specifications, hereinafter also referred to as "FO"). 4. The Ordering Party requires the following to be offered: a) medical devices within the meaning of the Act on Medical Devices (Journal of Laws 2024, item 1620, as amended), hereinafter referred to as the Medical Devices Act), placed on the market and used in the territory of the Republic of Poland in accordance with applicable regulations – for all tasks, all items (except for item 1 in task 8, where the Ordering Party requires the offer of a cosmetic). 5. The Ordering Party requires that the shelf life of all offered products be at least 12 months, counting from the date of delivery to the Ordering Partys registered office. 6. The quantities of products given in the unit price form are indicative and the Ordering Party has the right to order smaller quantities, but not less than 60% of the contract/task value. 7. Pursuant to Art. 30, section 2 of the Public Procurement Law, the Ordering Party awards the contract in lots, each of which is the subject of a separate procedure. 8. The order consists of 15 tasks, the detailed description of which is included in FCJ 9.
