The Independent Public Healthcare Facility of the Ministry of Internal Affairs and Administration in Katowice named after Sergeant Grzegorz Załoga in Poland from Europe region has released this tender for 5.1. Draft provisions of the contract – Appendix No. 2 to the Technical Specifications 5.4. The assortment and price form constitutes the content of the offer and does not need to be supplemented. Failure to submit the product and pricing form will result in negative consequences for the Contractor in the form of rejection of the offer pursuant to art. 226 sec. 1 item 5 of the Public Procurement Law. 5.5. The description of the subject of the contract and the proposed provisions of the contract should be read together with any possible changes to the content of the Technical Specifications, e.g., resulting from responses to Contractors inquiries. 5.6. Maximum delivery time: 5 business days from the date of order placement, during Ordering Partys business hours from 7:00 a.m. to 2:00 p.m. Order delivery after 2:00 p.m. will be treated as next-day delivery. Orders placed after 2:30 p.m. will be treated as next-day orders. 5.7. Required minimum shelf life of the delivered subject of the contract – at least 12 months, for package no. 3 – at least 3 years. 5.8. Offered medical devices must have a shelf life of at least 12 months from the date of delivery. The Ordering Party allows the delivery of the subject of the order with a shelf life of less than 12 months only if the Contractor obtains the Ordering Partys written consent. 5.9. The Ordering Party requires that the Contractor provide all catalog numbers/product codes of medical devices that will be subject to the tender agreement in the assortment and pricing form. with tender notice no. 715313-2026 published on 16 Oct 2026.
