Supplier Information Form (format provided as attachment to this EOI): Supplier full name, address, email address and telephone number Name and contact information of person submitting the proposal Full names of the company owners Payment information: bank name and address and demonstrated acceptance of the Syrian American Medical Society (SAMS) terms of payment as follows: i. 100% of payment value is made within 30 days of full acceptance of items delivered ii. Partial payments could be considered in exceptional basis and based on PO value Minimum 3 references with names and contact details Supplier Self-Certification of Eligibility.
